I confirm that I have disclosed all relevant health conditions, medications, and hair treatment history to my stylist at Unique Styles Hair and Esthetics.
I understand that a hair perm is a chemical process that alters the natural structure of my hair to create curls or waves, and results may vary based on my hair’s current condition, texture, previous chemical treatments, and at-home haircare routine.
I acknowledge that potential risks include, but are not limited to: dryness, breakage, scalp irritation, uneven curl pattern, allergic reactions, or the need for additional corrective services. I have been advised of the potential outcomes and risks associated with receiving a hair perm service.
I understand that proper aftercare is essential to maintain the integrity of my hair following the perm, and I agree to follow the home care instructions provided by my stylist.
I voluntarily consent to receive a hair perm service at Unique Styles Hair and Esthetics, and I release the salon, its stylists, and affiliates from any liability relating to this service.
Post-Treatment Care:
Following your perm, avoid washing your hair, tying it back, or using heat styling tools (e.g., flat irons, curling wands) for at least 48 hours to allow the curls to fully set. Use a sulfate-free, moisturizing shampoo and conditioner specifically designed for chemically treated hair. Regular deep conditioning is recommended to maintain hair health. Avoid excessive brushing or manipulation of curls, and protect your hair from excessive sun, chlorine, and saltwater exposure.
